P-Shot vs Penile Filler: What Each Does

Reviewed by Daniel Moghadam, M.D.5 min read
The short answer

They solve different problems. The P-Shot is platelet-rich plasma aimed at erectile function and does not change size. Penile filler adds girth and does nothing for erectile function. Confusing the two is the most common mistake men make in this field.

The Core Difference

P-Shot (PRP)Penile filler
TreatsErectile functionGirth / appearance
MaterialYour own plateletsDermal filler
Adds size?NoYes — circumference only
Improves erections?Possibly, modestlyNo
Onset12–24 weeksImmediate
DurationVariable, months12–24 months, or permanent
Reversible?N/AYes, if hyaluronic acid

Why This Confusion Exists

Marketing. The P-Shot is frequently advertised alongside enlargement services, and some clinics imply a size benefit that the procedure does not deliver. The published trials measure erectile function scores, not circumference.

The Google question "does the P-Shot increase size permanently?" is asked constantly, and the honest answer is that it is not a size treatment at all — so permanence does not apply.

How to choose

If your concern is how it looks — girth, appearance, proportion — that is filler.

If your concern is how it works — achieving or maintaining an erection — that is an erectile dysfunction evaluation first, then possibly the P-Shot or other treatment.

If it is both, they can be done, but the function workup should come first. A vascular cause found during that assessment matters more than either procedure.

Evidence, Compared

P-Shot: a 2025 meta-analysis of seven randomized trials (660 patients) found significant improvement in erectile scores at 12 and 24 weeks; a 2024 review of four studies found no conclusive evidence. Preparation varies widely between clinics. Small to moderate benefit.

Filler: a 301-patient comparison found girth gains of 2.1 to 2.9 cm depending on material, with adverse events in 7 to 14%. No FDA-approved product exists for this indication.

Neither is a settled, well-regulated field. Both deserve the questions in our safety article.

Can You Have Both?

Yes, though not usually in the same session, and the sequence matters. If erectile function is a concern, evaluate and address that first — partly because the workup may reveal something requiring medical management, and partly because it is difficult to judge satisfaction with appearance while function is a worry.

Dr. Daniel Moghadam will map a sequence rather than sell you both at once.

Related Questions

The P-Shot is not a size treatment, so permanence does not apply. It uses platelet-rich plasma to target erectile function. If size is your goal, penile filler is the relevant procedure.

They treat different things. The P-Shot uses your own platelets to target erectile function and does not change size. Filler adds girth by placing product under the skin and does nothing for erectile function.

If erectile function is any part of your concern, evaluate that first. The workup can reveal a vascular or hormonal cause that matters more than either procedure, and it is hard to judge satisfaction with appearance while function is a worry.

Yes, though usually not in the same session. We would sequence them, starting with the function assessment, rather than performing both at once.

No. Filler sits superficial to the erectile bodies and adds volume to circumference. It has no mechanism for improving erectile function, and any provider suggesting otherwise is overstating it.

Which one do you actually need?

A consultation sorts that out before you spend money on the wrong procedure.
