How Kybella Works

Kybella is synthetic deoxycholic acid — a molecule your body already makes to break down dietary fat. Injected into the pocket beneath the chin, it disrupts the membranes of fat cells, which are then cleared by your body's normal metabolic processes over the following weeks.

The important consequence: those fat cells are permanently destroyed. Adults do not generate meaningful numbers of new fat cells, so once the submental pocket is reduced, it stays reduced — provided your weight remains reasonably stable.

This is what separates Kybella from treatments that shrink fat cells temporarily. It is a structural change, not a maintenance program.

Why diet often fails here

Submental fullness is frequently genetic. Some people carry a defined fat pad under the chin regardless of body weight — which is why slim, fit patients are among the most common candidates. No amount of chin exercises will reduce it, because the issue is a fat compartment, not muscle tone.

What Treatment Is Actually Like

We will be straightforward: Kybella causes more swelling than most injectable treatments, and you should plan for it.

The injection itself takes fifteen to twenty minutes — a grid of small injections across the treatment area after numbing. Afterward, the area swells significantly. Expect noticeable swelling for several days, sometimes up to two weeks after the first session. Many patients schedule treatment before a weekend or plan a quieter week.

Swelling is not a complication here; it is the visible sign of the inflammatory process that clears the fat. Numbness, tenderness, firmness, and occasional bruising in the area are also normal and resolve over the following weeks.

Sessions

Most patients need two to four sessions spaced at least a month apart. Some see enough improvement after two; larger fat pockets may need more. Results build progressively, with visible change typically from four to six weeks after each session.

Is Kybella Right for You?

Good candidates have a distinct pocket of fat beneath the chin, reasonably good skin elasticity, and stable weight. The best results occur when the problem is genuinely fat rather than skin.

Kybella is not the answer when the fullness is caused by loose skin rather than fat — destroying fat beneath lax skin can make sagging more apparent. It also will not correct a profile whose real issue is a recessed chin; that is often better addressed with chin filler. If a prominent platysmal band or an enlarged salivary gland is contributing, Kybella will not help.

It is not appropriate during pregnancy or breastfeeding, or with an active infection at the site. Because injecting near the marginal mandibular nerve can cause a temporary asymmetric smile if placed incorrectly, anatomical precision matters — which is why Dr. Moghadam performs these injections personally.

Your Appointment, Step by Step

Assessment

Dr. Moghadam determines whether the fullness is fat, skin laxity, muscle banding, or a combination — and whether Kybella will actually help.

Mapping and numbing

The treatment area is marked with a grid and numbed with topical anesthetic, often with ice applied for comfort.

Injection

A series of small injections across the marked area. Fifteen to twenty minutes.

Swelling phase

Expect meaningful swelling for several days to two weeks. This is expected and reflects the fat-clearing process.

Reassessment

At four to six weeks we evaluate the result and decide whether another session is warranted.

Kybella Questions

Yes. Deoxycholic acid destroys the fat cells it contacts, and adults do not regenerate meaningful numbers of new fat cells. Once the submental fat is reduced it does not return, provided your weight stays reasonably stable. Significant weight gain can enlarge the remaining fat cells in the area.

Most patients need two to four sessions spaced at least a month apart. Smaller pockets sometimes resolve in two; larger ones may take four or more. Dr. Moghadam will give you a realistic estimate at consultation and reassess after each session.

More than with most injectables — this is the main thing to plan around. Significant swelling under the chin is normal for several days and can last up to two weeks after the first session. It is a sign the treatment is working, not a complication. Many patients schedule before a weekend or a lighter week at work.

There is a burning or stinging sensation during and shortly after injection, which typically settles within minutes to an hour. We use topical numbing and often ice to reduce it. Afterward the area is tender and may feel firm or numb for a few weeks.

No — Kybella removes fat, it does not tighten skin. If your skin has good elasticity it will often retract on its own as the fat reduces. If loose skin is the primary problem, removing fat beneath it can make sagging more noticeable, so we would recommend a skin-tightening approach such as a PDO thread lift or RF microneedling instead. The consultation determines which problem you actually have.

It is not appropriate if you are pregnant or breastfeeding, have an active infection at the treatment site, or if your submental fullness comes from loose skin, muscle banding, or an enlarged salivary gland rather than fat. Patients with a history of difficulty swallowing or prior surgery in the area should discuss this in detail beforehand.

Kybella is priced per vial, and how many vials you need depends on the size of the fat pocket and how many sessions you require. Because it is permanent, most patients view it as a one-time investment rather than an ongoing cost. We quote the full expected course at your complimentary consultation. Call (213) 895-4730.

Both reduce submental fat permanently but work differently. Kybella is injectable and conforms well to small, irregular pockets, with more swelling afterward. Cryolipolysis uses an applicator to freeze fat and generally causes less swelling but needs a pocket that fits the applicator. Dr. Moghadam will recommend based on the shape and size of your fat pad.
